Your Right to Receive a Good Faith Estimate
At Hamzavi Psychiatry, we are committed to transparency in healthcare costs and helping our patients understand the expected cost of their care.
Under the No Surprises Act, healthcare providers are required to provide patients who are uninsured or choosing not to use insurance with a Good Faith Estimate of expected medical charges for scheduled services.
A Good Faith Estimate explains the anticipated costs of psychiatric and mental health services before treatment begins.
What Is Included in a Good Faith Estimate?
Your estimate may include expected charges for:
- Initial psychiatric evaluations
- Medication management appointments
- Psychotherapy sessions
- Telehealth appointments
- Follow-up visits
- Additional recommended mental health services
The estimate is based on information known at the time it is created and may vary depending on your individual treatment needs.
Your Rights Under the No Surprises Act
You have the right to:
- Receive a written Good Faith Estimate before your scheduled service
- Ask questions about your estimated charges
- Dispute a bill if the final charges are substantially higher than the estimate provided
If you receive a bill that is at least $400 more than your Good Faith Estimate, you may be eligible to dispute the charges through the federal patient-provider dispute resolution process.
Requesting a Good Faith Estimate
Patients who are uninsured or not using insurance for services may request a Good Faith Estimate prior to scheduling or receiving treatment.
